Descripción
Asociación de hipoglucemiantes orales con mecanismos de acción complementarios. Vildagliptina es un inhibidor de la DPP-4 que aumenta el nivel de incretinas estimulando la secreción pancreática de insulina. Metformina aumenta la sensibilidad a la insulina del hígado y los tejidos periféricos.
Metformina disminuye la HbA1c (-1,5%) y las complicaciones macrovasculares y la mortalidad en pacientes diabéticos tipo 2, especialmente con sobrepeso (estudio UKPDS, Lancet 1998). La eficacia de vildagliptina se considera modesta, añadida a metformina redujo el HbA1c un 0,5% adicional en los ensayos clínicos. La combinación metformina + vildagliptina como terapia de inicio ha demostrado ser más eficaz en el control glucémico que la terapia secuencial primero con metformina y luego añadir vildagliptina (estudio VERIFY, Lancet 2019).
